    • At what age must we leave your practice and move to an adult-focused clinic?

      • Will you refer us to a new provider?
      • If you don’t refer to a new provider, will you make a recommendation for finding one?
      • How will you help our youth get ready to start seeing a new doctor?
      • Will you work with the new doctor so they can get to know our youth?
    • Some clinics will introduce you to your new adult doctor in the form of a transfer visit, a tour of the adult spaces, or in a planned transition visit. If none of these options are available remember, the first scheduled appointment with your adult provider is a meet and greet where you can get to know each other, usually referred to as “establishing care” in a new clinic.

    • When you are ready to move to an adult doctor some clinics will schedule a transfer visit. At a transfer visit you are usually encouraged to bring your parents or caregivers, yourself, and any questions you have about the transfer process. You usually get the chance to meet with your new doctor and some new team members during this visit, and your current doctor is typically present to help with the transition.

    • If you have a transfer visit your next visit is usually with your new adult doctor, and from then on you go to them for your care. If you do not have a transfer visit, you can expect that your pediatric doctor will tell you when you will no longer see them for your care. If you are concerned that you haven’t been given information on how or when to find a new doctor, we encourage you to ask what the next step is in this process.

    • You’re not alone! The thought of transitioning from your pediatric clinic can feel overwhelming. The good news is that there are resources to help support you along the way. Transition feels less scary when you start the conversation early, and you have a plan in place that you helped create.

    • Pediatric Care and adult care can be the same in some ways, but there are many differences that are important to learn about:

    • While every patient’s transition process is unique, ultimately every pediatric patient will transition to adult care at some point. Adult care providers understand how your conditions affect your adult body and are the best at helping you be confident and independent in taking care of yourself as an adult. When you move to an adult doctor, it makes room for new kids to be seen by pediatric doctors.

    • There is no perfect time to transfer, it depends on your readiness, and your clinic’s policy around transition. While most patients transfer around the age of 18-23, every clinic is different. The important thing is to focus on having conversations with your pediatric providers about planning for when you do transfer so you know what to expect and have a say in the process.

    • Start having conversations with your pediatrician sooner rather than later to allow enough time to find an adult doctor who is accepting new patients. If the time for transition comes and there is no adult specialty doctor available, the goal is to make sure you continue to receive quality uninterrupted care until you establish with one. This might mean remaining in your peds clinic temporarily, calling your insurance provider for assistance in locating other clinics, or utilizing urgent care until establishing new care.

    • Your pediatrician will likely have recommendations for adult doctors, and you can also use your insurance company to find adult providers in your area who are accepting new patients (provider finder on their website, or a call). Choosing a doctor is a personal choice, so recommendations are helpful, but you can always pick a different doctor.

    • Choosing a doctor is an important decision. You’ll want to establish care with a provider that you feel you can trust, respects your decisions about your health, and fits with your communication style. However, referrals to adult clinics can sometimes take a bit of time to process so we recommend staying with your existing provider until you have established care with a new one. This way, you can avoid a potential lapse in care.
